During the mid -day and mid-night pre-tour, I engaged the crew to refresh their minds on safe lifting operations and then shared video of an incident leading to a fatality while lifting crossover from the rig floor to the pipe rack which the crossover slipped from a casing strap, hitting a chief mechanic who was working below on the pipe rack.
Regarding lifting operations, we shared lessons about the importance of proper planning to make sure all equipment are certified and suitable for use and rated for the load; that all personnel are competent to perform their roles as riggers, banksman and crane operator; that risks have been assessed; landing area and pathway of the load is clear and secured; and need to stop when there is high wind speed including avoiding line of fire hazards such as not touching suspended load but to ensure good use of handsfree tools and avoiding getting under suspended load.
Regarding the incident leading to fatality, we looked at the importance of work authorization for simultaneous operations; the importance of situational awareness at all times; good housekeeping; avoiding short cut; using the right tool for the job (lifting cup for crossover) and the importance of communication.
